Hi everyone. I am a nursing manager in a NJ hospital and am looking for a little help with the Medication Reconciliation forms that is required by Joint Commission. Our institution developed a form but I am just not happy with it. It is not as efficent as I would like it to be. Can anyone tell me how they are complying with this Patient Safety Goal? We are not a computerized hospital. Any information would be appreciated.  |
